At least four vulnerable children were allowed to spend time with two killers.
Triple murderer Regina Arthurell was living with two children who were under the care of the state.
Arthurell, formerly known as Reginald, killed Thomas Thornton and Venet Mulhall.
Arthurell served 24 years for Venet’s murder and was released about six years ago.
NSW Department of Communities and Justice placed two vulnerable children in the home where Arthurell was living in NSW.
Athurell has also threatened Venet’s family after being released from prison.
Meanwhile, two children in the care of the state were found to be living with their grandfather who is a convicted killer.
“It should never have happened and I’m deeply apologetic for what has happened,” NSW Minister for Families and Communities Kate Washington told media.
It’s not known what is now in place from preventing this happening again.
